NEMT licenses: fees and processing times for each approval
Typical fees and processing times for the approvals an NEMT business needs, and how the slowest ones set your start date and the timing of your lease.
What to take away
- Typical application fees fall between about $25 and $2,000 per approval, and processing runs from a few days to six months.
- State passenger carrier authority has the widest range$100 to $2,000 plus per-vehicle fees, and three weeks to six months.
- Medicaid or broker enrollment costs $0 to a few hundred and takes 30 to 120 days, but it comes last, after insurance, permits and driver files.
- These are typical ranges, not quoted prices, so ask each agency for its current schedule in writing.
- Plan on two to six months from first call to first paid trip, and start the slow approvals before you sign a lease.
Fees and processing time for each approval
Four approvals carry both a fee and a wait, and the state carrier authority has the widest spread of each. The figures are typical ranges for a new NEMT business, not quoted prices, and your state may use a different label for the same approval.
| Approval | Issued by, typical | Typical application fee | Typical processing |
|---|---|---|---|
| Local business license | City or county | $50 to $500 a year | Days to three weeks |
| City vehicle permit, per vehicle | City or county | $25 to $500 a year | One to six weeks |
| State passenger carrier authority | State utilities commission or department of transportation | $100 to $2,000, plus per-vehicle fees | Three weeks to six months |
| Medicaid or broker enrollment | State Medicaid agency or its broker | $0 to a few hundred | 30 to 120 days |
Ask each agency for its current schedule in writing, because a figure printed on a forum is usually stale. Per-vehicle lines on the city and state rows grow with fleet size, so a second van adds to both.
Why the state row varies so much
The state row has the widest range because each state names and prices its own authority. California issues a TCP permit, Texas registers motor carriers by weight, and New York licenses for-hire vehicles in the city.
Buses in New York fall under State DOT operating authority, with Article 19-A driver certification. Ask each body to confirm its current name. Departments reorganize, and two counties inside one state can ask for different paperwork.
What sets your start date
The slowest row sets your start date, and it is usually the state authority or enrollment. Start both before you sign a lease, because rent paid while an application waits buys nothing. Plan on two to six months from first call to first paid trip.
Enrollment cannot be filed early. It comes once you can evidence insurance, permits and driver files, and applying first is how an application sits untouched for months. Federal law frames the benefit rather than issuing the license: 42 CFR 431.53 says a State plan must specify that the Medicaid agency will ensure necessary transportation for beneficiaries to and from providers.
Ask the state Medicaid agency
- How is NEMT delivered in this state?
- Directly administered or through a broker?
- If a broker, which one?
- What must a provider hold to enroll?
Five calls that fix the waiting time
Each call below returns a fee and a wait for one row of the table. Make them in this order, and write down the answer you get.
- State passenger carrier regulatorthe fee schedule and current processing time, before you buy a vehicle, because its answer changes which vehicles you may buy. The vehicles and premises guide walks through that decision.
- State Medicaid agencywhether you bill directly or enroll through a broker, and who holds the contract.
- The broker, if one existsthe provider enrollment pack and the per-trip documentation standard.
- City or countyvehicle permit fees per vehicle, the business license, and whether your yard is zoned for fleet parking.
- Insurance brokerthe cover and the certificate of insurance that the applications ask for.
Hold the enrollment filing until all five answers are in hand. The licensing and compliance guide maps which body governs which part of the work, so you know whom each question belongs to.
Paper that stops an application restarting
Applications ask for the same five sets of paper. Build the business registration set once and reuse every set for each filing.
- Businessarticles of organization, EIN letter, certificate of good standing, W-9.
- Insurancea certificate of insurance naming the agency or broker as certificate holder, with limits printed on it.
- Vehiclesregistration, title, VIN list, and lift or ramp inspection records.
- Driverslicense copy, motor vehicle record, background check result, and training certificates.
- Enrollmenta signed attestation against the OIG List of Excluded Individuals and Entities, a service area map, and an NPI where the state issues one.
The driver file is where most first applications stall. Where federal motor carrier rules apply, 49 CFR Part 391 says each motor carrier shall maintain a driver qualification file for each driver it employs.
Crossing a county or state line
Two extra approvals appear when your service area crosses a border. Carrying passengers for hire across a state line is interstate commerce, which brings an FMCSA operating authority and a USDOT number. Crossing into a neighboring county often triggers a second local permit.
Count both in your start date before you promise a service area. Check whether your passenger vans fall under federal rules at all. Many sit under the weight and seating thresholds, which changes your paperwork rather than removing it.

Common questions
Are the fees on this page what my agency will charge?
No. They are typical ranges, not quoted prices. Ask each agency for its current schedule in writing before you budget.
Do I need the licenses before I buy vehicles?
Get the state carrier answer first, because it decides vehicle class and inspection requirements. Buy after that answer, not before.
What if my state pays through a broker instead of direct enrollment?
Then the broker contract is your license to bill. Read the enrollment pack before you ask questions, because it answers most of them.
How long from first call to first paid trip?
Plan on two to six months. The slowest step is usually state authority or enrollment, so start both early and keep the written answers from every call in one file.
